| Welcome to the FS 5/7 Form Help Screen. This screen displays information from your previous entry of the control number and license plate number and allows you to enter the appropriate insurance information. The back button will return you to the Recertification of Liability Insurance screen. The submit button will take you to the confirmation page if there has been no lapse in your coverage and will move you to the Payment Option screen if there has been a lapse. The refresh button will reset your input and the cancel button moves you to the confirmation page.
